Healthcare spending within North Carolina's state prison system has surged dramatically over the past decade, placing significant strain on the Department of Adult Correction's budget. New data reveals a staggering 65 percent increase in these costs, a climb primarily fueled by the rapidly aging population behind bars.
Correctional facilities are increasingly functioning as geriatric centers, managing chronic and complex conditions like heart disease, diabetes, and dementia. This shift requires more frequent medical interventions, specialized medications, and adaptive equipment, all of which come at a high price.
Compounding the financial pressure are severe staffing shortages for medical positions within the prisons. This has forced the system to rely more heavily on outside community hospitals and specialists for inmate care, services that carry substantially higher price tags than in-house treatment. The reliance on external providers, while necessary, adds a major and unpredictable variable to annual budgets.
State officials acknowledge the trend is a direct reflection of longer sentences and an increase in older individuals entering the system. The escalating costs present a persistent challenge, demanding ongoing budgetary adjustments and strategic planning to meet the constitutionally required standard of care for a prisoner demographic that is growing older and sicker.
