Cityblock Health, known for its work delivering care to urban Medicaid and dual-eligible members, is acquiring Homeward Health in a move that significantly broadens its geographic footprint. The company also announced it has secured a new funding round of $116 million to support the expansion.
The combined organization will now serve close to 250,000 individuals, according to Cityblock CEO and co-founder Toyin Ajayi. In a blog post, Ajayi noted that the merger positions the company to reach the roughly 120 million people across the United States who rely on government-funded healthcare programs. This marks a strategic shift for Cityblock, which has historically concentrated its efforts in dense city environments.
Homeward Health brings experience in caring for patients in rural areas, often through a mix of in-person visits and virtual care. The acquisition is expected to allow both companies to share resources, data, and care models. Ajayi emphasized that the goal is to deliver better health outcomes for people who have limited access to traditional medical services, regardless of where they live.
The fresh capital, combined with the acquisition, signals growing investor confidence in value-based care models that focus on preventive services and managing chronic conditions. Cityblock plans to integrate Homeward's operations over the coming months while maintaining its existing partnerships with health plans and community organizations. The company says the move is not about expanding for its own sake, but about building a more complete system of care that meets people where they are, both physically and in terms of their health needs.
